The Queens EOC online application must be completed and submitted for admissions consideration to participate in any EOC program. The application can be completed in-person, with staff assistance as necessary, at the Queens EOC Admissions Office, located at 158-29 Archer Avenue, Jamaica, NY (between 160th Street and Parsons Blvd. above the MTA subways station where the E, J and Z lines end on Parsons Blvd. and Archer Avenue), Monday – Wednesday, 9:30 AM – 3:00 PM and Thursday, 11:30 AM – 4:00 PM. Visitors to the EOC are required to present valid, unexpired government-issued photo identification to enter the Queens EOC.
The online Queens EOC application can also be completed and submitted by visiting the Queens EOC web page at https://queenseoc.net and selecting the

button that is located on the right corner of the homepage. It will take you to the EOC application. Be sure to SELECT the Queens EOC as the EOC you want to attend, as well as your program choice from the drop-down menu. Please complete the entire application. It will start the admissions team working on your application right away (the online application is accessible 24/7).
